At the end of the module, the student possesses the knowledge relating to the methodology, organizational and clinical assistance models for taking charge, management and continuity of assistance in chronic situations. Develop knowledge related to therapeutic education. Knows the resources of the family and the community.

Frontal and interactive lesson with exercises and case analyzes.
Assessment methods
Written test unified medical area and chronicity + Oral:
Test unified: Care planning on a clinical case with identification of two nursing diagnosis and a collaborative problem complete with related objectives, indicators and interventions (preparatory for the oral exam) [30 minutes].
Oral: open questions on the chronicity program.
